I've noticed multiple video ended events before - but hadn't linked them
to anything specific (like ads).
I'd assumed it was related to something internal such as getting to the
end of the stream before hitting the time limit.
Personally, I wrap the state and make sure I only respond to the first
change.

The official process is to file an issue here with the label
API-YouTube:

The public tracker doesn't seem to be watched very closely though - so
reporting it here (as you've done) is normally the fastest option -
along with a starred issue if it's a feature request).
