[YouTube-API] Domain Blocked by YouTube

Hi,

A few months ago one of my websites Map of Metal (http://
www.mapofmetal.com) was blocked by Youtube. The site is fairly popular
and has been featured in numerous blogs, newspapers & radio stations.

Basically one day all videos returned error code 150 and failed to
load. At the time I realized that the site was using the api in a way
that was not within the TOS. However I quickly rectified any issues I
could find and migrated the container swf to a different domain so the
website is working now.

However I would like to have the blacklisting removed on the
mapofmetal.com domain. What is the best way to go about this and how
can I contact Youtube?

If any Youtube staff read this, please contact me as soon as
possible.
