I'm not really sure how to proceed without a reliable way to
reproduce the issue. There is a "Copy debug info" menu item when right-
clicking on the HTML5 player, which should bring up a dialog box with
text you could copy. If you could grab that when a playback failure
occurs on youtube.com I'll pass that along to the Players team, but I
don't know whether there will be much info there relevant to this
investigation.
